Step 1: Call to prequalify and set an appointment to come in and see the doctor
Step 2: Come in to see the doctor and talk about you medical condition. The doctor
will need to know if you are currently taking any medication. (If you are currently recieving treatment or have medical
records pertaining to your condition, please bring them with you)
Step 3: Upon Approval,
recieve your recommendation letter and ID card.
